It has been seven months since the beginning of the urgent and emergency care transformation in Dorset and we welcome you to join us at our second event in conjunction with the Dorset VCS Alliance to talk with us about how we work together in partnership.
There will be an opportunity to understand the progress of key programmes under the Our Dorset Transforming Health and Care Together banner, including Thriving Communities, Fulfilled Lives, Integrated Neighbourhood Teams and the FutureCare Programmes.
Attendees will be able to talk to the FutureCare Programme team, members of the Transfer of Care team, reablement providers, commissioners and other system leaders to share thoughts on how your organisation can contribute to FutureCare and play a bigger part in improving health and care outcomes for the residents of Dorset.
It will also be an opportunity to have in-depth discussions about key system and programme challenges including:
- Opportunities to learn more about joining a Thriving Communities Network
- NHS Dorset and FutureCare Commissioning Intentions for 2026/27
- Working together to provide better home-based intermediate care
- Supporting people who return home from hospital without a formal support package
- How we link existing VCSE services to the new Transfer of Care Hubs
- How we link work underway to help more people go home from ED without an admission
